How to access the service?
You can contact the IAPT Team directly to make a self-referral by calling or visiting our website.
Sunderland – 0191 566 5450
Others such as your GP can make a referral on your behalf if you prefer.
Someone from the team will then either make contact via the telephone or post to arrange an appointment with you for your initial assessment.
Before an assessment appointment takes place you will be asked to complete a PHQ9 and GAD7 form. This will give the practitioner an idea of how you have been feeling (with regard to your mood and anxiety levels) over the last 7 days.
This appointment usually takes place over the telephone (but can be face to face) and will last approximately 30 – 45 minutes. As part of the assessment, a member of the IAPT Team will discuss the best options for support with you.
